APPOINTMENT OF SERVICE PROVIDERS OR INDIVIDUALS TO BE PLACED ON A DATABASE TO MODERATE AND DEVELOP ASSESSMENT INSTRUMENTS FOR A PERIOD OF 5 YEARS FOR THE FOLLOWING OCCUPATIONAL QUALIFICATIONS:
· HOME BASED PERSONAL CARE ASSISTANT WITH SAQA ID 104779 AND 68 CREDITS
· HOME BASED PERSONAL CARE WORKER WITH SAQA ID 10792 AND 135 CREDITS
REQUIREMENTS:
- A degree in Public Health Care or related field of Home-Based Care
- A minimum of 3 years experience working within the Home-Based Care environment and or public/private health
- A minimum of 3 years in developing and moderating assessment instruments, facilitating or assessing.
- Three contactable references
- Be registered with the Health Profession Council of South Africa (HPCSA) or any other regulatory body where applicable in terms of qualifications
- Experience in working with the Department of Health or any civil society linked to the health/social services sector
QUALIFICATIONS REQUIREMENTS
- Degree in Public Health Care or Home-Based Care
- Assessor/Moderator certificate
SPECIAL REQUIREMENTS TO ACCOMPANY THE APPLICATION:
- Application letter
- Curriculum Vitae (CV)
- Three references on assessment instruments developed
- Qualification /Certificate in anatomy and physiology
- Proof of registration with the Central Supplier Database (CSD)
- A valid Tax pin
- A B-BBEE certificate
- An SBD 4 form
- Quotation of the required services
- An agreement between the Project Manager and the Director of the company

Note: that bids will be evaluated on functionality separately first. Bids scoring less than 70% on functionality will not be considered further in the evaluation and will be disqualified.
